New Patient Info

 

Equi is currently accepting new patients for all of our providers, which include Naturopaths, an Acupuncturist, and a Counselor. For more particular info, please see our providers page.  

 

Client Services is available Monday-Friday from 10am-6pm (closed from 1:30-2:30 for lunch.) Please check the available times and specifics for your provider, and if you are filling out paperwork ahead of time, be sure to complete the Equi intake form as well as any provider specific forms.

 

Our clinic is located inside the Portland Q Center at 4115 N. Mississippi Avenue. Please enter from Mississippi and check in with Client Services, and allow at least 10 minutes for paperwork. You are welcome to bring a partner, friend, parent or advocate. Interpretation services can be arranged for with notice ahead of time. Our building is ADA accessible, has gender neutral bathrooms, is on the #4 bus line, and has free onsite parking.

Dr. Angela Carter, N.D., Sacred Vessel Medicine

Your Health Goals are of utmost importance, as are your autonomy and your unique needs. Dr. Carter offers themselves first and foremost as an educator to help you learn how to better manage your health over the long term. To schedule with Dr. Carter, please call the office directly. 

 

New patient visits are scheduled for 60 minutes.

Dr. Carter will thoroughly review your health concerns, health history, and family history, and discuss your diet, lifestyle, and health habits with you.

 

They will then conduct appropriate physical examinations pertaining to your needs and health conditions.
You will receive an individual treatment plan to start you on your way to better health.

 

Follow up visits are approximately 15-45 minutes in length.

Dr. Carter will review your treatment plan with you and make any adjustments necessary to continue you on you path to health.

 

Appointments with Dr. Carter available:
Tuesday: 10 a.m. - 6 p.m.
Wednesday: 10 a.m. - 6 p.m.
Friday: 10 a.m. - 6 p.m.

 

Payment options for Dr. Carter:

They are on panel with the following insurance companies: OHP, Kaiser, Health Share/Care Oregon, Family Care, Optum/United, Cigna, MODA, Lifewise and Blue Cross Blue Shield. They also offer a financial hardship rate of $30 for folks who are low income and/or uninsured (on a limited basis.) Please see our payments and policies page for more info. 

 

Forms for Dr. Carters patients: These are in pdf format and can be downloaded, printed, and brought in with you for your appointment in lieu of coming early to fill them out in the office. The privacy policy does not need to be printed, but it does need to be read thoroughly before you sign the sheet saying you read it and agree to the terms. If you have a current or previous doctor you'd like to request files from, please also print and fill out the record request form. If you have any questions, please contact us.

Rowan Everard, LAc

Rowan is passionate about community healing, and specializes in treating psychological trauma, chronic pain, and urogenital disorders.  To schedule an appointment with Rowan, please email him directly at: rowan.everard@gmail.com.
